Rise > 26.4 μmol/L or 150 to 200% of baseline

<0.5 ml/kg/h for >6 h

Rise of 200 to 300% of baseline

<0.5 ml/kg/h for >12 h

Increase of 300% or creatinine >354 μmol/L with acute rise of at least 44 μmol/L

<0.3 ml/kg/h for >24 h or anuria for 12 h
